    The Discharge Planning Assistant performs clerical and coordination of post hospital transitions of care ordering and notifications for Care Coordinators and Social Workers as part of the hospital’s care coordination department. This role works directly with the Care Coordinators and Social Workers in the daily discharge planning functions and implementation of orders for transitions of care.  As part of the overall case management team, this role works closely and cooperatively with the all members of the care coordination department.   


    • Assists staff with the coordination of transitions to post-acute facilities, ordering equipment (DME), and making referrals as directed.
    • Performs all clerical functions related to insurance authorization and Post-Acute Care providers.
    • Interacts with community partners to provide order specifics for transitions of care post hospitalization and provide information for authorizations.
    • Accepts telephone, fax and electronic requests for post-acute services and refers to the most appropriate providers in a timely manner. Responsible for accurate and timely entry of information related to orders and providers into the appropriate information system.
    • Maintains documentation and files in an organized manner to allow access of information by appropriate individuals.
    • Communicates authorizations to Care Coordination Nurses and documents them in the EMR to facilitate timely discharge planning.
    • Works collaboratively with the Care Coordinators (as directed) to assist them in contacting post-acute providers and confirming elements of successful patient transition to the next level of care.
    • Responds rapidly to requests for Durable Medical Equipment, Infusion Therapy, Medical Supplies, Home Health services, Skilled Nursing Facilities and any other post-acute providers within the community.
    • Supports nThrive’s Compliance Program by adhering to policies and procedures pertaining to HIPAA, FDCPA, FCRA, and other laws applicable to nThrive’s business practices. This includes: becoming familiar with nThrive’s Code of Ethics, attending training as required, notifying management or nThrive’s Helpline when there is a compliance concern or incident, HIPAA-compliant handling of patient information, and demonstrable awareness of confidentiality obligations.


    • High school diploma or GED.
    • At least one year of experience working in an acute care hospital or community provider.
    • Must have computer knowledge and knowledge of electronic medical record system such as Powerchart.
    • Ability to multi-task and solve unique problems with insurance coverage authorization and post-acute providers in a creative and independent manner.
    • Knowledge of Medicare and Medicaid payment rules, policies and regulations.
    • Ability to demonstrate knowledge of medical terminology.
    • Ability to evaluate medical records and other health care data.
    • Ability to establish and maintain effective and cooperative working relationships with hospital staff and others contacted in the course of this position.
    • Demonstrated ability to effectively present information and respond to questions from small groups or on a one-on-one basis.
    • Proven time management skills.
    • Demonstrated experience communicating effectively with a customer and simplifying complex information.
    • Ability to handle sensitive information and maintain HIPAA compliance.
    • Demonstrated ability to navigate Internet Explorer and Microsoft Office.

    Preferred Skills

    • Experience with insurance and patient contact.
    • Bachelor’s degree in health related field such as social work or Health Sciences.
